In lieu of all other central matters at hand, a completely separate point of order you wish to discuss is either prefixed or suffixed with "otherwise". What is usually being proposed is in fact a non-sequitur of not very much finess. The gesture is nevertheless a polite one. And that's what is important.

Doctor: "Ok, ok. That full arm we will amputate no problem. Otherwise any pain?"
